New Patient Information
We are happy you chose us to provide your care. To provide the best care, to assist you with payment from your insurance company, and to comply with regulations, we need you to complete some paperwork prior to your evaluation.

If you are a woman who will be treated for pelvic pain, or urinary incontinence or leakage, click on Download Women's Health New Patient Packet (PDF 284k size).

To complete the forms, print them, fill them in, and bring them with you to your first appointment.
When you arrive for your appointment please be wearing loose comfortable clothing. If you can, wear clothing that allows exposure of the primary area of complaint, for example, shorts if you have a knee problem, a halter top if you have a shoulder problem. We do also have shorts, t-shirts, and gown tops as needed. If you have a foot or ankle problem bring shoes you wear casually or, if in sports, bring your sports shoes.

On your first visit, we will get information from you and evaluate you so we can determine what problems you have and what treatment to use to correct the problem. We begin treatment on the first day, always if you are in pain. We will also have you complete a computer survey called FOTO, which provides a scientifically valid functional score based on the information you provide. Please arrive at your appointment 20 minutes early on your first visit to allow your record to be put together for your therapist if you have completed the paperwork or 30 minutes early if not. After that, please be on time for your appointments to get the most from treatment. Your appointment time is reserved specifically for you. If you are late, we will not extend your appointment time, and keep the next patient waiting.

If you must cancel an appointment, we require 24 hours notice. We always have patients who need those time slots. Your courtesy provides us the opportunity to notify them of the opening.

WE DO NOT BELIEVE THAT NO PAIN IS NO GAIN! Our goal is to relieve pain, not cause it. Some have the misconception that physical therapy hurts. That is rarely the case! We work with you and teach you how to move without causing pain.

New Patient Forms
Prior to your first appointment, please print all Standard Forms, except Notice of Privacy Practices. Complete the forms and bring them with you on your first visit. On the Consent Form is a place to sign that you have read and understand our Privacy Practices, which you should read online or print if you desire. Please arrive at your appointment 15 minutes early with your paperwork complete.
